This Isn't Your Mother's Pelvic Floor! - Women's Fitness, Pregnancy ...

elizabethPelvic floor health is crucial.? As a mother of nine and an exercise enthusiast, I realize that probably one of the most important things that I can do is to strengthen my pelvic floor muscles. For women especially, the pelvic floor should always be thought of as key to the core muscles.? While? it is far too under-emphasized, it has gained attention since our mothers were giving birth.? For that we should be grateful!? I remember being told by my ob/gyn, Dr. Tommy Megremis, after the birth of my sixth child, that I should seek out the help of a physical therapist who specialized in pelvic floor therapy because I was having symptoms of a weak pelvic floor.?? I actually had no idea what he was talking about! ?I had figured that I would simply have to bide my time before a surgical procedure would become a necessary and ?normal? part of the aging process.? I am grateful to Dr. Megremis for sending me in a better direction, and after working for about 4-5 months with the physical therapist, I really noticed a change.? I was so encouraged.? Since that time I have twice more sought the help of a physical therapist for pelvic health (Barbara Green) and each time felt stronger for it.? Often women ask me what to do about issues having to do with pelvic health and I am always amazed how few know about the physical therapy option.? In fact, sports physical therapist and owner of Architech Sports , Alan Tyson, believes that just like other areas of the body that often need strengthening and rehabilitation, the pelvic floor should not be neglected, and he tells me he wishes more women would seek therapy even as soon as after their first child is born!

There are two physical therapists who are bringing attention to this topic by sharing up-to-date information and providing a wonderful source for all things having to do with pelvic health. Stephanie A. Prendergast (MPT) and Elizabeth H. Rummer (MSPT), who work at Pelvic Health Rehabilitation Center (PHRC),? write at the blog? Pelvic Health and Rehab.? Their responses to my questions might surprise you! You will notice that I?add a few comments of my own in purple after some of their responses. ?I know you will find the interview interesting?

Whitney: I appreciate you answering my questions, Liz and Steph.?Tell me about PHRC!

PHRC is one of the very few physical therapy clinics that focuses solely on pelvic floor physical therapy. We espouse a multidisciplinary approach where we work closely with other providers to develop and implement treatment plans for our patients. The providers we commonly work with include ob/gyns, urologists, urogynocologists, colorectal specialists, pain management physicians, sex therapists, psychologists, among others. And since we?re the provider that?s going to be spending the most time with the patient, we tend to take a leadership role in the patient?s treatment, acting as CEO, if you will, of their overall healing process. ?

stephanieLastly, it?s important to us at PHRC to play a role in the education arena of the pelvic pain PT community as well as in the conversation about pelvic floor health in general. Toward that end, we teach a class titled ? Demystifying Pudendal Neuralgia as a Source of Pain: A Physical Therapist?s Approach? four times a year in different cities across the U.S.; are active in the International Pelvic Pain Society (IPPS)?Stephanie is the first PT ever to serve as the IPPS president?, the American Physical Therapy Association; and the World Congress on Pelvic Pain.

Whitney: What are your backgrounds?

Stephanie received her master?s in physical therapy at the Medical College of Pennsylvania and Hahnemann University in Philadelphia. She learned about myofascial pelvic pain at the start of her career, became intrigued, and chose to dedicate her life to helping men and women suffering from these syndromes. In addition to her involvement with the IPPS and World Congress on Pelvic Pain, she recently traveled to China to help begin educating the Chinese medical community on pelvic pain treatment.

For my part, I (Liz) received my master?s in physical therapy at the University of Miami Medical School. I am a Pennsylvania native, but I soon became a San Francisco convert after moving to the city more than a decade ago. One of the things I love to do is community outreach/education here in SF. For the past year or more, I?ve held regular workshops around the city on pelvic floor-related topics, including pain and sex and postpartum health/rehab. ?

Whitney: What made you decide to start this blog? What are you hoping to share with men and women?

For ?years Steph and I have gotten emails and phone calls from people dealing with pelvic pain as well as the providers struggling to treat them. As I?m sure you?re aware, pelvic floor health and rehab is relatively under-explored territory within the medical community. While it?s come a long way in the past decade?in just the past five years even!?it remains an under-researched and misunderstood area of medicine.

Meanwhile, there are as many people suffering with pelvic pain/dysfunction as there are suffering with back pain! So while the numbers of sufferers are quite high, many struggle to find the proper diagnosis and treatment and?quite frankly, just accurate information. So we thought that a blog would help us do what we were already doing, trying to give guidance to people navigating pelvic pain, but on a more widely accessible scale.? (Whitney): I am so happy to know about your resource. As one who writes about fitness and likes to share information with others, it is wonderful to find a reliable source of up-to-date wisdom regarding pelvic health!

Historically, across the board all women were being advised to do kegels. For the most part, this is still very much the case. But here?s the thing: the only women that should be doing kegels are women who have a truly weak pelvic floor.

There are a variety of reasons that the pelvic floor can become weak; childbirth and aging are two of the most commonly discussed. However, the fact of the matter is that not everyone who has children will develop a weak pelvic floor and not everyone is going to ultimately be stuck with a weak pelvic floor due to the aging process. ?

So to tell all women across the board to do kegels is bad advice.

For one thing, women who have tight pelvic floors/and or trigger points within their pelvic floor muscles who do kegels could end up doing harm to their pelvic floors. In these situations kegeling could cause these women to present with pain or if they already have pain, it could worsen their pain.

For another thing, studies show that the majority of women who are told to do kegels without the proper instruction on how to do them, don?t do them correctly anyway.

And lastly, kegels will not prevent the pelvic floor from becoming weak. They can, however, when done correctly, help to strengthen an already weakened pelvic floor.

So, to summarize, the current conversation is incorrect: not everybody across the board needs to do kegels?just women who have a truly weak pelvic floor. And the fact of the matter is, and I know this sounds self-serving, but it is the hard truth: the only way to tell if you have a truly weak pelvic floor is to go to a PT for an evaluation. Unless, you?ve been diagnosed as having a prolapse by a physician. In that case, your pelvic floor is obviously weak. (Whitney): I was actually told by my PT that I need to be very careful when I do kegels.? In the past I have done them incorrectly (basically ?pushing out? on those muscles instead of ?pulling up?).? Now I do my kegels when I am on the floor stretching.? If I had not been told how to properly do a kegel, I would probably be negating any positive effects of doing them at all.

Whitney:? After the birth of my sixth child, I went to a physical therapist who specialized in pelvic floor muscles?and I was amazed at the difference after a few months of working with her. Biofeedback and strengthening exercises helped tremendously?is that your primary mode of rehab?

That?s wonderful that you found relief through PT! We love success stories!

To answer your question: the short answer is no. The mode of rehabilitation we deploy depends entirely on what the patient?s findings are when we evaluate him or her.

If the problem is a weak pelvic floor with poor motor control then yes, strengthening exercises are likely among the treatment methods we will deploy. But, at PHRC it?s our preference not to use a Biofeedback machine. We prefer to use manual strategies to help our patients regain motor control and strengthen their pelvic floors. That?s not to say there is anything wrong with using a Biofeedback machine, we just prefer manual, hands on treatment approaches.

Say the patient comes to us with incontinence as her (or his) primary symptom. We wouldn?t automatically assume that weakness is the issue because the fact is that there are a variety of issues beyond weakness that can cause someone to suffer with incontinence. So we would have to do a thorough evaluation to determine exactly what is causing the patient?s incontinence before we would determine what treatment approach to take. So the underlying reasons for the symptoms are what dictate our treatment approach?in every situation. There is no one-size-fits-all treatment protocol for pelvic floor problems. Each case is unique and requires a tailored treatment plan.

For instance, you?ll notice that in the first part of my response I said that strengthening exercises would most likely be what we would deploy if a patient had a weak pelvic floor. I was careful to leave wiggle room because it?s possible for a patient to present with both a weak pelvic floor and an overly tight pelvic floor with trigger points. In a circumstance such as this strengthening runs the risk of causing the patient pain. So, most likely we would have to take care of the tightness and trigger points before we could strengthen. And that?s another thing I should point out: as the patient progresses in treatment, the plan will change and evolve.

Lastly, another reason we wouldn?t depend on strengthening and biofeedback as our primary mode of rehab is that we also see patients with pain, not just weakness. And the treatment for pain is the opposite of the treatment for weakness. Pain is typically caused by a too-tight pelvic floor and/or trigger points within the pelvic floor muscles. Treatment for those impairments would involve lengthening the muscle and releasing the trigger points.

So, again, the mode of rehab depends entirely on what we find in our evaluation and consider to be the underlying reasons for the patient?s complaints.

Whitney:? I have had nine children and no C-sections?is it inevitable that I will deal with problems like incontinence or prolapse?

The good news is no. There really is no hard and fast way of predicting if an individual will end up with incontinence or prolapse.

That said, there is research that shows that if a women has any level of incontinence during pregnancy than she will be more likely to suffer with it after delivery. But other than that, just because you have one child, or nine for that matter, does not mean you?ll inevitably have to deal with incontinence or prolapse.

mother with babyMH900407140On the flip side, there is research that was conducted abroad in places where women are given pelvic floor rehabilitation after pregnancy and delivery that showed that as a result of the intervention, the incidence of incontinence was lowered. (France is perhaps the country most well-known for its postpartum pelvic floor rehab.)? (Whitney): Alan Tyson, sports PT, has told me that he believes pelvic floor rehabilitation should begin right after birth and should be a focus of the postpartum care with women being referred to a PT who specializes in the pelvic floor. I wish I had not waited until after my sixth child was born!

Whitney: What should we be doing to help ourselves to avoid these things?

Female doctorMH900409503My best advice is that at some point in a woman?s life, whether it?s after having a child (or after each child) or when she?s reached middle age, she should see a pelvic floor PT, or in the case of prolapse a urogynocologist, and have what is, for all intents and purposes, a pelvic floor check up. The provider will be able to tell her if there is weakness and/or prolapse. If there is an issue or the beginnings of an issue, than getting evaluated at this point will be proactive in the sense that there can be some sort of intervention, which could fix the problem so that future symptoms/issues don?t arise or are not as serious if/when they do.

Here?s the thing: in our society we are accustomed to being proactive about certain health issues? colon cancer, breast cancer, our oral health?so it?s not unprecedented for a woman to be proactive in this way about preventing future problems with her pelvic floor. As women are becoming more and more aware of their pelvic floors my hope is that they will begin to go for a pelvic floor check up at some point, and in that way give themselves a chance of avoiding future issues. (Whitney):? I hope so, too!

Whitney: I read an article awhile back at the blog Katysays about squats and I noticed that you also commented on that article in your blog. ?Since reading that article, I try to do squats fairly regularly. Do you also recommend squats for women?

It sounds as if you are talking about squatting to release your pelvic floor. If that?s the case, then yes, squats to release your pelvic floor can be helpful if you are someone dealing with an overly tight pelvic floor. This is a way to relax your pelvic floor. However, if you are someone dealing with nerve-like symptoms, such as burning, shooting, or stabbing pain in your pelvic floor region, I don?t recommend this position because it stretches the pudendal nerve, which in turn can cause irritation.

Other strategies to achieve pelvic floor relaxation are flat back pelvic floor drops and diaphragmatic breathing. However, if your goal is to strengthen your pelvic floor, none of these exercises will achieve this.

Whitney:? If someone has already had a prolapse and undergone surgery, what kinds of things can she do to strengthen the pelvic floor?

There isn?t really a laundry list of exercises I can give you here because it?s not a good idea to do pelvic floor strengthening exercises in this situation without being under the supervision of a doctor or PT. That?s because before you do pelvic floor strengthening you want to know exactly what?s going on with your pelvic floor.

Whitney:? If a woman is having symptoms of a prolapse, would you advise her to seek a surgical option?

It all depends on the level of the prolapse. And only a physician can determine this. However, there are levels of prolapse that can be controlled with less invasive methods, such as use of a pessary. In our book, the less invasive the option, the better as any kind of pelvic floor surgery can cause scar tissue to form, which in turn can open the door to a whole new set of issues. (Whitney):? inline MB900422402Whitney: ?Are there certain sports or fitness routines that you tell women to avoid if they have weak pelvic floor muscles?things like tennis, running, plyometrics, jump rope exercises?

The exercises you mention do indeed cause stress to the pelvic floor muscles and if they are truly weak can further weaken or irritate already existing symptoms. (Whitney): I have gladly given up many plyometric type exercises that involve jumping, however running will always be a cherished form of fitness for me and I hope to continue it for some time to come!

Whitney:? If there is one thing women should be doing to help themselves to avoid future problems having to do with a weakened pelvic floor, what would it be?

Here my answer is the same as with the question about about avoiding incontinence and prolapse. At some point/points, if there is concern or if a woman has just had a baby, going for a pelvic floor check up is the best way to avoid problems that could arise because of weakness. Going for a pelvic floor check up can result in existing problems being caught and corrected before getting worse. Another course of action might be to see a pelvic floor PT before delivery if you are pregnant and are already dealing with any incontinence issues. (Whitney): I would have to agree.? The wealth of information, strategies, and new techniques and exercises that were introduced to me by Barbara Green (PT) were tremendously helpful and I am grateful that I have found her!

Climate change blamed for Australia's extreme weather

The east coast of Australia has been drenched by floods and torrential rains, even as recent bush fires affecting much of the country continued to burn. Four people are known to have died as Australians get a further taste of extreme weather that is predicted to become more common as the planet warms.

The deluge came as a storm that started as tropical cyclone Oswald just north of Australia was dragged south over most of the east coast by a low-pressure system extending all the way to New South Wales, says Richard Wardle of the Bureau of Meteorology in Queensland. As it hit land, Oswald lost its cyclone status but remained a "vigorous" storm, Wardle says.

With no low-pressure zone further east to pull Oswald out to sea, the storm stayed over land, moving slowly south and dumping huge amounts of rain on coastal communities. Bundaberg, a town in Queensland, experienced its worst-ever flood as the storm lingered nearby for nearly 24 hours, leading to the evacuation of 7500 people from their homes. In Brisbane, the floods were almost as bad as those that devastated the city two years ago.

Climate change to blame

In Queensland and New South Wales, the deluge arrived while the bush fires that broke out two weeks were still smouldering. At the time, the Bureau of Meteorology said that the exceptionally hot, dry weather that led to the fires was "consistent" with climate change. Experts are now drawing the same conclusions about the rains.

"The frequency of more intense events is going to increase. Droughts, heatwaves and ? in northern Australia ? rainfall events and tropical cyclones are going to be more intense," says Jon Nott of James Cook University in Townsville, Australia, who researches extreme weather events.

Nott says that more intense rainfall in the tropics and subtropics is one of the things we can expect with global warming. The connection between tropical cyclones and climate change is complicated: fewer cyclones are expected, but the ones that strike will be more severe. They could also become 20 per cent wetter.

Nott points out that Australia might be experiencing a "double whammy" of climate change and natural variability, driving wetter conditions. One natural pattern, the Interdecadal Pacific Oscillation, affects circulation in the Pacific, and reverses every 20 or 30 years. It flipped about five years ago for the first time since 1977, bringing warmer waters to Australia's east coast. "During those phases, Queensland sees more flooding, more rainfall, and more landfalling tropical cyclones," Nott says. Climate change will only compound the effects of such patterns, he says.

Is Your Job Search Like a Bad Super Bowl Commercial? | The ...

Bad-Super-Bowl-CommercialEvery year, big brands bring together the best minds in advertising. Their mission: to create one television commercial good enough to air during the Super Bowl of advertising: the Super Bowl.

To ensure they make the ?Best of?? lists that appear on Monday, advertisers plug in as many pop references and celebrities as possible. After months of effort, the spots are presented to the ?judges? on the world?s stage, 30 seconds at a time.

For some, the investment is well worth the effort ? their brand gets a huge boost in recognition and sales; some even rise to the top. Others spend a lot of time, energy and money ? and fail to impress? sometimes miserably. (Remember Groupon?s ads from a couple years ago?)

How does this scenario compare to your job search?

You spend months perfecting on your resume, often bringing in the best minds within your sphere of influence (usually a significant other, roommate or parent). You scour the web for resume templates and accompanying cover letters for what you believe the audience will most appreciate ? complete with buzzwords and influential name-drops. When you?re sure your presentation is complete, you submit to the judges: the gatekeepers, recruiters and human resources.

After all this effort, if the judges don?t view you as the best ? and place the other contenders well above you on their version of a ?Best of?? list ? you?ll need to try again. And again. And again.

To carry this analogy further?

Do you think the companies whose commercials get widely panned this year will use the same agency and talent for next year?s Super Bowl? Not likely. They?ll bring in new creatives, experts and support teams. They?ll do much better research on what works and what doesn?t. They?ll learn from their mistakes.

They won?t fail again.

If your job search is struggling, and you consistently find yourself falling short as a would-be contender, you may want to take the same approach:

  • Using someone other than your significant other, roomie or mother for review ? repackage your resume, cover letter and online presence
  • Consult a professional career expert to help with your interview techniques and to introduce best practices that work in today?s competitive job market
  • Before submitting even one more application, research the company and the hiring manager/recruiter ? learn what works for them, and what doesn?t ? and customize your message accordingly; perhaps even build a networking relationship with someone already working at the company
  • Quit beating your head against the wall hoping ?this one will be different? ? it won?t, until you make it different ? by learning from your mistakes

Don?t go into your next ?big game? with yet another message that won?t appeal to the audience. Prepare now ? start over if necessary, again and again. The next time you interview, you just may find yourself on the ?best? list.

For Early Cancer, Lumpectomy Beats Mastectomy for Survival: Study ...

MONDAY Jan. 28, 2013 -- Breast-conserving surgery for early stage breast cancers may result in better survival than mastectomy, according to a new study.

For those with early stage breast cancer, "lumpectomy is just as effective if not more effective than mastectomy," said researcher Dr. Shelley Hwang, chief of breast surgery at Duke Cancer Institute in Durham, N.C.

"There are lots of women who think the more [treatment] they do, the better they will do," she said. "This refutes that."

The findings, published online Jan. 28 in the journal Cancer, are especially strong for women over 50 with hormone-sensitive cancers, the researchers found.

Earlier research had also concluded that the two procedures are similarly effective, but Hwang's is a more "real-world" study.

Hwang's team looked at 14 years of data from the California Cancer Registry, following more than 112,000 women with early stage breast cancer (stages 1 or 2) between 1990 and 2004. Ages ranged from 39 to 80.

More than half (55 percent) had lumpectomy and radiation, while 45 percent had mastectomy (complete breast removal) alone.

Hwang compared lumpectomy and radiation with mastectomy alone, not mastectomy plus radiation. "We wanted to look at early stage disease, and those patients typically don't get radiation after mastectomy," she said.

The researchers tracked the women's progress for a median of more than nine years (half followed longer, half less). During that time, more than 31,000 women died, nearly 40 percent of them from breast cancer. The others died of other causes.

For the first three years after treatment, those who had a mastectomy had a higher risk of dying from heart disease and other ailments than those who had lumpectomy. This may indicate that the women who underwent lumpectomy were generally healthier, Hwang said.

Over the entire follow-up, those who underwent lumpectomy were more likely to survive the breast cancer.

"The group that benefited the most -- who had the biggest difference in breast cancer survival -- were those women over 50 with estrogen-receptor positive disease," Hwang said. This means their cancer depends on estrogen to grow.

Among those women, the lumpectomy group had a 13 percent lower risk of death from breast cancer and a 19 percent lower risk of death from any cause than those who had a mastectomy.

Not all women with early stage breast cancers can have a lumpectomy, Hwang said. In this procedure, just the tumor and some healthy tissue are removed, sparing the rest of the breast. Among the exceptions are those whose cancers are too large, or those who have different cancers in the same breast.

The percent of women with early breast cancers choosing a mastectomy has risen recently, after a dip in previous years. Hwang and others suspect that women told they could safely opt for lumpectomy were still afraid to try it.

The new research, which was funded by the U.S. National Cancer Institute, suggests that if a lumpectomy is possible, it may actually increase survival, Hwang said.

The findings may reverse the mastectomy trend, said Dr. Laura Kruper, co-director of the breast oncology program at the City of Hope Comprehensive Cancer Center in Duarte, Calif., who was not involved in the study.

The study is scientifically sound in many ways, Kruper said. "They broke it down by year of diagnosis and by age category," she said. "They looked at socioeconomic status, and they kept it early stage."

Dr. Wendy Woodward, section chief for breast radiation oncology at the University of Texas MD Anderson Cancer Center in Houston, said that for women with early cancers, the study "clearly reiterates there is no detriment to cancer control in having a lumpectomy and radiation for breast-conserving surgery candidates."

But, Woodward added, "I am not sure the study convinces us that lumpectomy and radiation is better for breast cancer survival, but it may be."

The study was observational, Hwang stressed. It found a link or association but could not provide cause-and-effect proof that the breast-conserving treatment is more effective than mastectomy in early stage breast cancer.

Hwang believes the study does arm women with valuable information. However, "I don't want women who chose mastectomy to think they did the wrong thing," Hwang said. "At the end of the day, personal preference trumps everything else. I fully support the patient's options to choose the best treatment for themselves."

US home prices accelerate in November

(AP) ? U.S. home prices accelerated in November compared with a year ago, pushed higher by rising sales and a tighter supply of available homes.

The Standard & Poor's/Case-Shiller 20-city home price index rose 5.5 percent in November compared with the same month a year ago. That's the largest year-over-year gain in six years.

All but one of the cities in the index posted annual gains. The largest gains was in Phoenix, where prices jumped nearly 23 percent. It was followed by San Francisco, where prices rose 12.7 percent, and Detroit, where prices increased 11.9 percent.

Prices also rose in 10 of the cities measured by the index in November from October. That's up from seven in October from September. The biggest monthly gains were in San Francisco, Phoenix and Minneapolis.

Monthly prices are not seasonally adjusted and frequently decline over the winter. The 20-city index dipped in November from the previous month.

Steady price increases should help fuel the housing recovery. They encourage more people to buy before prices rise further. Higher prices also build homeowners' wealth, which can spur more spending and economic growth.

"Housing is clearly recovering," David M. Blitzer, chairman of the Case-Shiller index, who noted that prices are rising for both new and previously occupied homes.

Purchases of previously occupied homes rose last year to their highest level in five years. The National Association of Realtors forecasts that sales will rise 9 percent this year. Independent economists have similar forecasts.

Sales of new homes also rose in 2012, although they remain near depressed levels.

Stable job gains and record-low mortgage rates have encouraged more people to buy homes. And the limited inventory of homes for sale has made builders more confident to step up construction. The number of previously occupied homes has fallen to an 11-year low.

Millions of homeowners still owe more on their mortgages than their homes are worth, making it difficult for them to sell. But higher home values are lowering the numbers of those "under water" and should encourage more homeowners to sell.

More people are also moving out on their own after living with friends and relatives in the recession. That's driving a big gain in apartment construction and also pushing up rents. That's also encouraging investors to buy homes and rent them.

The tighter supply of homes pushed builders in December to start work on the most homes in 4 ? years. Last year was the best year for residential construction 2008, just after the recession started.

Toward 2-D devices: Single-atom-thick patterns combine conductor and insulator

Jan. 27, 2013 ? Rice University scientists have taken an important step toward the creation of two-dimensional electronics with a process to make patterns in atom-thick layers that combine a conductor and an insulator.

The materials at play -- graphene and hexagonal boron nitride -- have been merged into sheets and built into a variety of patterns at nanoscale dimensions.

Rice introduced a technique to stitch the identically structured materials together nearly three years ago. Since then, the idea has received a lot of attention from researchers interested in the prospect of building 2-D, atomic-layer circuits, said Rice materials scientist Pulickel Ajayan. He is one of the authors of the new work that appears this week in Nature Nanotechnology. In particular, Ajayan noted that Cornell University scientists reported an advance late last year on the art of making atomic-layer heterostructures through sequential growth schemes.

This week's contribution by Rice offers manufacturers the possibility of shrinking electronic devices into even smaller packages. While Rice's technical capabilities limited features to a resolution of about 100 nanometers, the only real limits are those defined by modern lithographic techniques, according to the researchers. (A nanometer is one-billionth of a meter.)

"It should be possible to make fully functional devices with circuits 30, even 20 nanometers wide, all in two dimensions," said Rice researcher Jun Lou, a co-author of the new paper. That would make circuits on about the same scale as in current semiconductor fabrication, he said.

Graphene has been touted as a wonder material since its discovery in the last decade. Even at one atom thick, the hexagonal array of carbon atoms has proven its potential as a fascinating electronic material. But to build a working device, conductors alone will not do. Graphene-based electronics require similar, compatible 2-D materials for other components, and researchers have found hexagonal boron nitride (h-BN) works nicely as an insulator.

H-BN looks like graphene, with the same chicken-wire atomic array. The earlier work at Rice showed that merging graphene and h-BN via chemical vapor deposition (CVD) created sheets with pools of the two that afforded some control of the material's electronic properties. Ajayan said at the time that the creation offered "a great playground for materials scientists."

He has since concluded that the area of two-dimensional materials beyond graphene "has grown significantly and will play out as one of the key exciting materials in the near future."

His prediction bears fruit in the new work, in which finely detailed patterns of graphene are laced into gaps created in sheets of h-BN. Combs, bars, concentric rings and even microscopic Rice Owls were laid down through a lithographic process. The interface between elements, seen clearly in scanning transmission electron microscope images taken at Oak Ridge National Laboratories, shows a razor-sharp transition from graphene to h-BN along a subnanometer line.

"This is not a simple quilt," Lou said. "It's very precisely engineered. We can control the domain sizes and the domain shapes, both of which are necessary to make electronic devices."

The new technique also began with CVD. Lead author Zheng Liu, a Rice research scientist, and his colleagues first laid down a sheet of h-BN. Laser-cut photoresistant masks were placed over the h-BN, and exposed material was etched away with argon gas. (A focused ion beam system was later used to create even finer patterns, down to 100-nanometer resolution, without masks.) After the masks were washed away, graphene was grown via CVD in the open spaces, where it bonded edge-to-edge with the h-BN. The hybrid layer could then be picked up and placed on any substrate.

While there's much work ahead to characterize the atomic bonds where graphene and h-BN domains meet and to analyze potential defects along the boundaries, Liu's electrical measurements proved the components' qualities remain intact.

"One important thing Zheng showed is that even by doing all kinds of growth, then etching, then regrowth, the intrinsic properties of these two materials are not affected," Lou said. "Insulators stay insulators; they're not doped by the carbon. And the graphene still looks very good. That's important, because we want to be sure what we're growing is exactly what we want."

Liu said the next step is to place a third element, a semiconductor, into the 2-D fabric. "We're trying very hard to integrate this into the platform," he said. "If we can do that, we can build truly integrated in-plane devices." That would give new options to manufacturers toying with the idea of flexible electronics, he said.

"The contribution of this paper is to demonstrate the general process," Lou added. "It's robust, it's repeatable and it creates materials with very nice properties and with dimensions that are at the limit of what is possible."

New look at cell membrane reveals surprising organization

Jan. 28, 2013 ? Sight would dramatically alter a blind man's understanding of an elephant, according to the old story. Now, a look directly at a cell surface is changing our understanding of cell membrane organization.?

Using a completely new approach to imaging cell membranes, a study by researchers from the University of Illinois, Lawrence Livermore National Laboratory and the National Institutes of Health revealed some surprising relationships among molecules within cell membranes.

Led by Mary Kraft, a U. of I. professor of chemical and biomolecular engineering, the team published its findings in the Proceedings of the National Academy of Sciences.

Cells are enveloped in semi-permeable membranes that act as a barrier between the inside and outside of the cell. The membrane is mainly composed of a class of molecules called lipids, studded with proteins that help regulate how the cell responds to its environment.

"Lipids have multiple functions serving as both membrane structure and signaling molecules, so they regulate other functions inside the cell," Kraft said. "Therefore, understanding how they're organized is important. You need to know where they are to figure out how they're doing these regulatory functions."

One widely held belief among cell biologists is that lipids in the membrane assemble into patches, called domains, that differ in composition. However, research into how lipids are organized in the membrane, and how that organization affects cell function, has been hampered by the lack of direct observation. Although the cell membrane is heavily studied, the imaging techniques used infer the locations of certain molecules based on assumed associations with other molecules.

In the new study, Kraft's team used an advanced, molecule-specific imaging method that allowed the researchers to look at the membrane itself and map a particular type of lipid on mouse cell membranes. The researchers fed lipids labeled with rare stable isotopes to the cells and then imaged the distribution of the isotopes with high-resolution imaging mass spectrometry.

Called sphingolipids (SFING-go-lih-pids), these molecules are thought to associate with cholesterol to form small domains about 200 nanometers across. The direct imaging method revealed that sphingolipids do indeed form domains, but not in the way the researchers expected.

The domains were much bigger than suggested by prior experiments. The 200-nanometer domains clustered together to form much larger, micrometer-sized patches of sphingolipids in the membrane.

"We were amazed when we saw the first images of the patches of sphingolipids across the cell surface," said Peter Weber, who directed the team at Lawrence Livermore National Laboratory. "We weren't sure if our imaging mass spectrometry method would be sensitive enough to detect the labeled lipids, let alone what we would see."

Furthermore, when the researchers looked at cells that were low on cholesterol -- thought to play a key role in lipid aggregation -- they were surprised to find that the lipids still formed domains. On the other hand, disruption to the cell's structural scaffold seemed to dissolve the lipid clusters.

"We found that the presence of domains was somewhat affected by cholesterol but was more affected by the cytoskeleton -- the protein network underneath the membrane," Kraft said. "The central issue is that the data are suggesting that the mechanism that's responsible for these domains is much more complicated than initially expected."

In addition, the new study found that sphingolipids domains were incompletely associated with a marker protein that researchers have long assumed dwelled where sphingolipids congregated. This means that data collected with imaging techniques that target this protein are not as accurate in representing sphingolipid distribution as previously thought.

"Our data are showing that if you want to know where sphingolipids are, look at the lipid, don't infer where it is based on other molecules, and now there's a way to directly image them," said Kraft, who also is affiliated with the department of chemistry at the U. of I.

Next, the researchers plan to use the direct-imaging method in conjunction with other more conventional methods, such as fluorescence, to further determine the organization of different kinds of molecules in the membrane, their interactions and how they affect the cell's function. They plan to begin by targeting cholesterol.

"Cholesterol abundance is important," Kraft said. "You change that, you tremendously change cell function. How is it organized? Is it also in domains? That's related to the question, what's the mechanism responsible for these structures and what are they doing?"

UFC on Fox 6 results: Demetrious Johnson retains title, ?Rampage? loses in Octagon finale

CHICAGO -- The UFC's stop in the Windy City had two great knockouts, a champion holding onto his belt, and the last fight of a one-time champ.

Despite a strong start by John Dodson, Demetrious Johnson held onto his championship belt with a unanimous decision. The judges saw it 48-47, 49-46, 48-47 for Johnson.

Unsurprisingly, Johnson and Dodson fought a fast-paced first two rounds. Johnson tried to slow things down in the first round with a takedown. While he did get Dodson to to the ground, the challenger popped back to his feet quickly.

Dodson dropped Johnson twice in the second round, and shook off Johnson's take down attempts with a great sprawl. Johnson seemed to have a hard time even getting close to Dodson to land a punch.

A Johnson knee in the fourth round caused a small fight stoppage. Dodson's hand was on the ground as Johnson threw a knee to Dodson's head, which is an illegal strike. The bout was stopped as doctors checked Dodson's eye and Johnson was warned. The fight went on, but not without plenty of boos from the crowd in Chicago.

When the fight restarted, it was all Johnson. He controlled Dodson against the cage, and threw knees that busted up Dodson's face. Johnson was able to get a takedown at the beginning of the fifth round, too. Though boos rained down, Johnson kept the fight against the cage.

Later, the crowd got behind him as Johnson elevated, tightened his legs around Dodson's torso and threw elbows. It was a creative move that likely could only happen in the flyweight division. Johnson finished the round with Dodson against the cage, and knee after knee after knee to the body and face.

Quinton 'Rampage' Jackson vs. Glover Teixeira

If this really was Quinton "Rampage" Jackson's last fight in the UFC, it wasn't one to remember. He was outstruck and outwrestled by Glover Teixeira throughout their bout. Teixeira took the fight 30-27, 30-27, 29-28 on the judges' cards.

Teixeira's first round was his best, as he was able to take Jackson down early and mount him and take his back. Late in the round, Teixeira knocked Jackson down with a punch and looked close to finishing, but wasn't able to end the fight.

The rest of the fight featured a worn out Jackson trying to avoid Teixeira's takedown attempts unsuccessfully. Jackson threw some big punches, and even jawed at Teixeira during the bout, but he wasn't able to score any big offense.

While this fight will be remembered for being Jackson's last UFC bout, Teixeira scored his third straight UFC win on Saturday night. He looked impressive against the former champ.

Anthony Pettis vs. Donald Cerrone

Anthony Pettis won his much anticipated bout with Donald Cerrone by landing big, memorable strikes. Pettis was getting the better of their striking exchanges early, which set him up for a big finish. Pettis threw a kick that landed hard on Cerrone's body, then finished with a punch. Cerrone fell to the ground in a heap, and the fight was stopped at 2:35 in the first round.

Pettis lost his first fight in the UFC after coming from the WEC as the champ, but he's been winning ever since. He has wins over Joe Lauzon, Jeremy Stephens and now Cerrone. After the fight, he made a pitch to UFC president Dana White.

"I want my title shot. I should have had it years ago. People say I can?t wrestle because of the Guida fight, but I had a serious shoulder injury. Now I am 100 percent. No one can do in the Octagon what I can do."

While lightweight champion Benson Henderson has a fight set up with Gilbert Melendez, a shot for Pettis isn't unwarranted. It would be a rematch of their WEC title fight, which Pettis won in the promotion's final fight.

Ricardo Lamas vs. Erik Koch

Lamas won the first round by constantly pressuring Koch against the cage. It wasn't exactly the most thrilling round to have start a card on network television, but it led to a thrilling second round. Lamas took advantage of Koch's slip, and then finished the fight with nasty, nasty ground and pound. Lamas started with elbows, then started with strikes that cut open Koch's face. Koch had no answer for Lamas' relentless strikes, and the bout was stopped at 2:32 in the second round.

Lamas thought his performance warranted a title shot.

"I?ve beat Cub Swanson, I?ve beat Hioki and now I?ve beat Koch. All those guys were supposed to be fighting for the title at one point. I beat them all. We?re all here to be world champ and I am no different. I want the winner of Aldo vs Edgar next week. I don?t care who wins, I want the winner."

Clegg slams EU vote, polls show boost for PM

LONDON (Reuters) - Nick Clegg, leader of the junior party in the ruling coalition, denounced David Cameron's pledge to hold a referendum on quitting the European Union, as polls on Sunday indicated the prime minister's move may gain him votes.

"It is not in the national interest when we have this fragile recovery," said Clegg, whose Lib Dems strongly favour closer EU ties, in contrast to many members of Cameron's Conservative party. "I don't think it helps at all."

He dismissed as "implausible" Cameron's plan to take back powers from Brussels before a referendum on a new treaty by 2017 that would let voters take Britain out. EU leaders have shown little wish to grant Cameron concessions and Clegg said EU talks would distract ministers from efforts to revive the economy.

Cameron, he told the BBC, would damage economic growth if he spent "years flying around from one European capital to the next, fiddling around with the terms of Britain's membership".

The Lib Dems are languishing in the polls and are unlikely to leave the coalition before an election in 2015, but the EU issue has added to strains. Cameron, who says he wants Britain to stay in the EU, last week promised a referendum if he is re-elected. It is less clear what may happen if treaties remain unchanged.

The first opinion polls published since he made his pledge of an "in-out" vote, however, showed that the prime minister may be succeeding in reversing a drift from the Conservatives to a party which campaigns for Britain to leave the European Union.

A Survation poll in the Mail on Sunday, which showed Labour unchanged and in the lead on 38 percent, put the Conservatives on 31 percent, up two points, while the UK Independence Party was down by the same margin, on 14 percent. UKIP's surge from just 3 percent in the 2010 election has raised the prospect of a split on the right that could condemn Cameron to defeat.

Another poll, by ComRes in the Independent on Sunday, showed an even more marked "Brussels bounce" for the prime minister, with the Conservatives gaining five points from last month to 33 percent and UKIP losing four points to be on 10 percent. Again, ComRes put Labour in the lead, down a point on 39 percent.

Cameron's European move worries the United States and EU allies, which want Britain to stay in the bloc. Many business leaders say it creates dangerous uncertainty.

Many Conservatives, whose party toppled previous premiers over European policy, welcomed a referendum after 2015. However, without improvement in an economy which shrank by 0.3 percent in the last quarter, Cameron's re-election is far from certain.

There was a man on the radio this week talking about his sextant ? which was over 100 years old when he bought it some time ago. This was the only navigation instrument available to sailors for over a century. When searching for info for this entry I found a reference to Robin Knox Johnson?s circumnavigation of the globe 44 years ago ? he had no radio, a barometer from a pub and a sextant. How things have changed! GPS, now on most mobile phones can locate your position to within a metre or so.

Sextants rely on the knowing the exact time to calculate the position in fact the British admiralty sponsored the development of the first accurate timepiece that could go to sea ? pendulum clocks aren?t much use in a seaway! The balance wheel was the solution to this problem.

GPS rely on timing, too, but rather more precisely that a maritime chronometer. The basis for GPS is a series of satellites that orbit the earth ? not in synchronous orbits like communications satellites ? but with a period of just under 12 hours. These are equipped with extremely accurate atomic clocks. A GPS receiver will receive signals from at least four satellites and will calculate the distance from each based on the time take for the signal to reach the receiver from the satellite. It then calculates the position based on a mathematical principle called trilateration. This involved calculating the intersecton of the four or more spheres that are defined by the timings. Thinking of this in 3 dimensions is quite tricky (I always have to get an orange out when I try to think of the geometry of the earth!) In two dimensions, it?s a little simpler: here?s a local example. Suppose I?m 2.81 miles from Gt Missenden, 2.59 miles from Wendover and 3.33 miles from Princes Risborough. The first distance gives me a circle.?

Now try to imagine this in three dimensions ? intersections of spheres. The calculation to achieve a position is pretty complicated. GPS devices can also calculate the position in a number of formats including the GB OS grid reference. I?ve put a map on the waymark outside outside our house with a grid reference read from my GPS. Lots of walkers stop to look ? but I?m not sure if the reference means anything to many of them!
